Linkage to care and support is the cornerstone of effective health and wellness and HIV management. The foundation focuses on strengthening the healthcare infrastructure to ensure timely diagnosis, access to antiretroviral therapy, and ongoing support services for individuals living with HIV.
We work closely with healthcare providers, community-based organizations, and government agencies to improve the continuum of care, streamline referral systems, and enhance coordination among various stakeholders. By establishing a robust network of care and support, we can improve health outcomes and quality of life for our target population.
Our medical team provides regular check-ups, antiretroviral therapy, and education on living with HIV. These services are offered in an environment where they feel comfortable and free from stigma and discrimination.
When necessary, we also provide X-rays and conduct outreach programs to identify and link individuals to care, even in the comfort of their own homes. This includes, but is not limited to, monitoring responses to treatment, disease progression, and the delivery of medications.
This care is provided free of charge, regardless of your status in the TCI.
